Birth Certificate 

We will make a copy and return the original to you. You may also bring in a copy if that's all you have.

Immunization Record

Bring your record to be transcribed to the school form.

Proof of Residence

Guardianship Status

If applicable, please bring any documents showing that you are the student's legal guardian.

Request for Records

Please bring the complete address and fax/office number of your child's previous school.
