Got a Kindergartner?

To register your kindergarten student, complete and submit the Kindergarten Registration Packet.
All forms can be found below. Please note, the registration packet has both English and Spanish copies.

Welcome to Holt Elementary Registration

Please fill in and print all 4 forms that are highlighted in blue and bring them to the school when registering.

Please also bring with you:

Birth Certificate, parent ID, current immunization record, and 2 proofs of residency.

 These forms will need to be filled out and printed and brought to the school.
Birth Certificate (Original)  We will make a copy and return the original to you. 
Immunization Record Bring your record to be transcribed to the school form.
Proof of Residence Fill out top part and submit, then bring two proofs of residency with you.
Guardianship Status If applicable, please bring any documents showing that you are the student's legal guardian.
Request for Records Please bring the complete address and fax/office number of your child's previous school.
