Welcome to Holt Elementary Registration

Please fill out and print all 4 forms highlighted in blue and bring them with you to the school.

Please also bring with you:

Birth Certificate, parent ID, current immunization record, and 2 proofs of residency.

 These forms need to be filled out and printed and brought to the school when registering.
Birth Certificate  We will make a copy and return the original to you. You may also bring in a copy if that's all you have.
Immunization Record Bring your record to be transcribed to the school form.
Proof of Residence Click for acceptable proof of residence documents.
Guardianship Status If applicable, please bring any documents showing that you are the student's legal guardian.
Request for Records Please bring the complete address and fax/office number of your child's previous school.
